This series consists mostly of survey forms sent out to ASDC’s members in 1981. The survey covers questions on members’ personal information and information on their children and school, including ages, cause of deafness, and forms of communication used at home and in school. The survey also asks members to explain their reason for being an ASDC member and the services from the group they use. The series includes some sheets tabulating responses to the survey.

Also present are a few completed forms from a similar 1987 survey. The later survey is more concerned with ASDC’s services and what members find useful or want to see.
